Why is there a new consent form?

When research studies are changed or expire, the consent form may be updated to include new information that is important to you as a participant. Changes may clarify current study procedures, add new procedures, and provide information about changes in the risk of participation or design of the study.

Why do I need to sign the updated consent form?

The updated consent form is provided to let you know of any changes to the study. The changes won’t take effect until you have provided your informed consent. Signing the updated consent form indicates that you understand what you are being asked to do in the study and want to continue participation.

What happens to my data if I don’t sign this new consent form?

If you don’t sign the new consent form, collection of your data will continue until your initial consent expires or you withdraw from the Study.
